Uncertain future, financial situation not stable, significant other is not happy, I am not happy, old habits seem to be creeping up again.

Not sure anyone here is truly qualified to help you. Seeking out somebody who can or somebody who has helped in the past might be a good start.
I can offer that things are rarely as bad as they seem. Employment/ financial wise, just keep looking for something different and better. Keep letting people know that you are looking. I am guessing that your old habits won't help your financial situation or your relationship situation, so why would you want to do that?
Significant other is not something I want to comment on other than to say that you can typically improve relationships in your life if you improve yourself. Be the best version of you and he/ she will probably notice it.
