I guess it's quite possible that he never bothered to investigate whether their medical staff was worth a crap.  He probably didn't TRY kill anyone....unless the practice film is being used a snuff film or something...but that doesn't seem too plausible.

Pinkel comes across as a guy who is just not all that engaged in detailed matters. 

I think all of the "attention to detail" blah, blah that we hear about OB is mucho  , but I can't imagine OB would allow a situation like this to happen.
