This makes me wonder how a couple would react if the husband actually found the wife on Ashley Madison and they agreed to meet up at a hotel or something. So many ways that could go.  I wonder if it's ever happened?

Do you always use the same fake name? If so, then your wife would probably recognize it if she saw it on Ashley Madison.
