Fill this out.  Put that you didn't cancel your contract because of the cancellatin fee.  Celebrate with your $35 (for each line) at the bar with a toast to sd for telling you about it.

Also, if you have a first gen ipod nano you should fill this out too.  Repeat the toast to sd as applicable.
